A psychoeducational evaluation is a comprehensive assessment, completed by a licensed psychologist or LSSP, that identifies how a student learns best. It typically measures cognitive abilities (reasoning, memory, processing), academic skills (reading, writing, math), and, when indicated, attention, executive functioning, and social-emotional factors. Evaluations should be updated every three years to inform any changes to supports. This documentation helps our Success Center create a tailored Student Success Plan, similar in purpose to a Section 504 plan, so students receive the right accommodations and strategies. At St. Mark, a Student Success Plan (SSP)  is a personalized roadmap that supports students with current formal documentation (e.g., psychoeducational evaluations or relevant medical reports). Its purpose is to remove barriers, build on strengths, and ensure access to grade-level curriculum.

We develop each SSP by bringing together:

• Findings from the student’s formal documentation
• Curriculum benchmarks and assessment data
• Input from parents and teachers

As a team, we use this information to identify the accommodations and strategies a student needs to be successful. Those supports are then documented in the SSP and shared with the student’s teachers

• Intake & Plan Setup: For new families, we will meet before school begins (or as soon as possible if you enroll midyear), with the Success Center team to review your child’s documentation and draft a Student Success Plan (SSP). In the first few weeks of school, for both newly enrolled and established families, we’ll hold a brief follow-up to gather the teacher’s initial feedback and finalize the plan.
• Progress Check-Ins (optional): Midyear and end-of-year, or anytime by request, to review what’s working and make adjustments.
• Year-End Planning: Near the end of the school year, we set a preliminary plan for the following year, guided by feedback from you and your child’s current teacher as to what supports worked well and what will need to be adjusted.

All year long, we stay in close communication, monitor progress, and adjust the plan as needed to partner with your family to help your child thrive.

All 1st through 8th grade applicants must complete St. Mark’s enrollment process and be offered admission to be eligible for participating in the Success Center. In addition, placement in the Success Center depends on available space in both the Success Center and the relevant general education classrooms so we can implement support effectively. Placement is finalized only after these pieces are confirmed.

Once your completed application and supporting documentation are received:

1. Application & Documentation Review
   Our Admissions team and the Success Center Director will review your materials for program fit and current capacity.
2. Capacity Check
   We confirm space in the Success Center and in the grade-level/general education classrooms needed to support your child’s plan.
3. Follow-Up Contact
   We’ll reach out to confirm receipt, request any missing items, and outline next steps.
4. Scheduling
   If moving forward, we’ll schedule a time for your child to be tested and also be interviewed.
5. Decision & Onboarding
   We’ll communicate admission and placement decisions. If accepted and placed, we begin onboarding and set up your child’s Student Success Plan (SSP).

If space is not immediately available, we’ll discuss waitlist options and share expected timelines.
